Job summary

Corewell Health in Grand Rapids, MI, is seeking a licensed social worker (LLMSW or LMSW) to join an interdisciplinary team addressing complex psychosocial needs. You will assist with discharge planning, resources, and family support.

Responsibilities include assessments, referrals, and collaboration with community agencies, plus documentation in the EMR and short-term therapeutic support as needed. A Master’s in Social Work and Michigan MSW licensure on hire are required; 2 years of experience

Qualifications

• Master's Degree in Social Work is required.
• 2 years of relevant healthcare experience post-degree preferred.
• MSW licensure in Michigan upon hire required.
• Care management experience preferred.
• Experience in crisis intervention, discharge planning, and bereavement support is preferred.

Responsibilities

• Conduct psychosocial assessments with patients and families and develop an appropriate plan.
• Coordinate with alternate sites of care and community agencies.
• Document assessments and plans accurately in the electronic medical record.
• Provide consultation and resources to the healthcare team.
• Maintain knowledge of community resources and regulations.
• Participate in department or regional committees as assigned.
• Provide short-term therapeutic support as appropriate.
