Job summary

Qualifications

• Experience in individual, family assessment, crisis intervention, grief and loss counseling, and discharge planning preferred.

Responsibilities

• Conduct psychosocial assessments with patients and families to identify needs and plan post-discharge care.
• Communicate with alternate sites of care and community agencies to address patient needs.
• Document assessments and plans in the electronic medical record reflecting current condition and transition of care needs.
• Provide consultation and resources to healthcare team and maintain knowledge of community resources.
• Participate in departmental, regional, or statewide stakeholder activities as assigned.
