Corewell Health in Grand Rapids seeks a licensed social worker to collaborate with an interdisciplinary team to support patients and families with complex psychosocial needs, discharge planning, and transitions of care within the Palliative Inpatient - Hospice West department. The role requires a Master’s in Social Work and MI licensure upon hire; experience in crisis intervention, grief counseling, and assessment is preferred.

Typical hours are 8:00 a.m.

Formación

• Master's Degree in Social Work required.
• Experience in individual/family assessment, crisis intervention, grief counseling, and discharge planning preferred.
• LIC-MSW/LSW in Michigan upon hire required.

Responsabilidades

• Conduct psychosocial assessments with patient and family to identify needs and plan.
• Communicates with alternate sites of care and community agencies.
• Documents assessment and plan in EMR reflecting patient condition and transition needs.
• Provides consultation and resources to the healthcare team.
• Maintains knowledge of community resources.
• Participates in department/state/regional committees and projects as assigned.
• Develops and maintains knowledge of federal and state regulations as they pertain to the role.
