Job summary

Corewell Health in Lansing, MI is seeking a part-time Social Worker to join an interdisciplinary team. You will assist patients and families with complex psychosocial needs, coordinating with alternate sites of care and supporting discharge planning.

The role involves psychosocial assessments, crisis intervention, grief and loss counseling, and connecting patients with community resources. MSW is required; 2 years healthcare experience preferred, with opportunities to participate in committees

Qualifications

• Master's Degree Social Work is required.
• MSW license or eligibility required by the State of Michigan upon hire.
• Two years of relevant health care experience post-degree preferred.
• Care management experience preferred.
• Experience in individual/family assessment, crisis intervention, grief and loss counseling, and discharge planning preferred.

Responsibilities

• Conduct psychosocial assessments with patient and/or family to identify needs and plan for alternate site of care post discharge.
• Communicate with alternate sites of care and community agencies to address patient needs.
• Complete and document assessments and plans in the EMR reflecting condition, factors, and transition needs.
• Provide consultation and resources to the healthcare team.
• Maintain knowledge of community resources relevant to the role.
• Participate in department, regional, or statewide committees or projects as assigned.
• Maintain knowledge of federal and state regulations related to the role.
• Provide short-term therapeutic support as appropriate for setting and location.
