Qualifications

• Master Social Work LTD - State of Michigan Upon Hire required

Responsabilités

• Collaborate with an interdisciplinary team to assist patients and families with complex psychosocial needs and discharge planning
• Coordinate with alternate care sites and community agencies to address patient needs
• Document assessments and plans in the electronic medical record reflecting current condition and transition needs
• Provide consultation and resources to the healthcare team
• Maintain knowledge of community resources and participate in stakeholder committees as assigned
• Develop and maintain knowledge of federal and state regulations as they pertain to the role
• Support alignment with Corewell Health’s care management processes and patient-centered goals
