Job summary

Hometown Urgent Care is seeking a compassionate and skilled Primary Care Provider/Advanced Practice Provider (NP or PA) to deliver comprehensive primary care across the lifespan in a collaborative, team-based setting.

You will conduct comprehensive exams, diagnose and manage a wide range of conditions, and educate patients on prevention and treatment plans while collaborating with ENT, surgical and medical subspecialties. E-ClinicalWorks experience is a plus.

Qualifications

• Certified NP-C/FNP or PA-C licensed in Ohio.
• Experience in primary care, family medicine, or internal medicine.
• Comfortable managing a broad patient panel across ages and backgrounds.
• Proficient in outpatient clinical procedures.
• Strong communication and empathy to connect with patients.
• Commitment to teamwork, continuous learning, and high-quality care.
• E-Clinical Works EMR experience a PLUS!

Responsibilities

• Conduct comprehensive physical exams, assessments, and patient histories.
• Diagnose and manage a wide range of conditions.
• Provide primary care support across multiple specialties.
• Perform in-scope procedures.
• Collaborate with ENT, Surgical, and Medical Subspecialties to ensure continuity of care.
• Utilize E-ClinicalWorks for documentation.
• Provide emergency and urgent interventions when necessary.
• Educate patients and families on treatment plans and prevention.

• Educate patients and families on treatment plans, prevention strategies, and lifestyle modifications

• CertifiedNurse Practitioner (NP-C/FNP) or Physician Assistant (PA-C)licensed in the state of Ohio
• Must be experienced in primary care, family medicine, or internal medicine
• Comfortable managing a broad patient panel across all ages and backgrounds
• Proficient in clinical procedures relevant to outpatient care
• Strong communication and empathy skills—your ability toconnectwith patients matters as much as your clinical expertise
