Hometown Urgent Care & Occupational Health is seeking a compassionate Advanced Practice Provider (NP or PA) to deliver comprehensive primary care across all ages in a collaborative team setting. You will perform exams, manage diverse conditions, and support multiple specialties while coordinating with other clinicians.

Ideal candidates hold Ohio NP-C/FNP licensure and have experience in family or internal medicine.

Qualifications

• Certified Nurse Practitioner (NP-C/FNP) licensed in Ohio.
• Must have primary care, family medicine, or internal medicine experience.
• Comfortable managing a broad patient panel across all ages.
• Proficient in outpatient clinical procedures.
• Strong communication and empathy skills are essential.
• Commitment to teamwork and continuous learning.
• E-Clinical Works EMR experience is a plus.

Responsibilities

• Conduct comprehensive physical exams, assessments, and patient histories.
• Diagnose and manage a wide range of conditions.
• Provide primary care support across multiple specialties.
• Perform in-scope procedures.
• Collaborate with ENT, Surgical, and Medical Subspecialties for continuity of care.
• Utilize E-ClinicalWorks for documentation.
• Provide emergency and urgent interventions when needed.

Requirements
