Hometown Urgent Care is seeking a compassionate and skilled Advanced Practice Provider (NP or PA) to deliver comprehensive primary care across the lifespan in Streetsboro, Ohio. Join a collaborative, team-based environment focused on prevention, diagnosis, treatment, and education.

You will perform exams, manage conditions, and support care across specialties, with emphasis on strong patient communication, empathy, and high-quality outpatient procedures.

Qualifications

• Licensed NP-C/FNP or PA-C in Ohio.
• Experience in primary care, family medicine, or internal medicine.
• Comfort with a broad patient panel across all ages.
• Proficient in outpatient clinical procedures.
• Strong communication and empathy to connect with patients.
• E-Clinical Works EMR experience is a plus.

Responsibilities

• Conduct comprehensive physical exams, assessments, and patient histories.
• Diagnose and manage a wide range of conditions.
• Provide primary care support across multiple specialties.
• Perform in-scope procedures.
• Collaborate with ENT, Surgical, and Medical Subspecialties to ensure continuity and coordination of care.
• Educate patients and families on treatment plans, prevention strategies, and lifestyle modifications.

• Provide emergency and urgent interventions when necessary


• Educate patients and families on treatment plans, prevention strategies, and lifestyle modifications

• CertifiedNurse Practitioner (NP-C/FNP) or Physician Assistant (PA-C)licensed in the state of Ohio


• Must be experienced in primary care, family medicine, or internal medicine


• Comfortable managing a broad patient panel across all ages and backgrounds


• Proficient in clinical procedures relevant to outpatient care


• Strong communication and empathy skills-your ability toconnectwith patients matters as much as your clinical expertise
