Job summary

Hometown Urgent Care & Occupational Health in Ohio seeks a compassionate Advanced Practice Provider (NP or PA) to deliver comprehensive primary care across all ages. You will conduct exams, diagnose and manage conditions, and support multiple specialties in a team-based setting.

Join a collaborative practice with emphasis on prevention, education, and patient-centered care. Ohio NP-C/FNP licensure and outpatient procedures experience are required; E-ClinicalWorks experience is a plus.

Qualifications

• Must hold NP-C or FNP certification and an active Ohio license.
• Experience in primary care, family medicine, or internal medicine.
• Comfort managing a broad patient panel across ages.
• Proficient in outpatient clinical procedures.
• Excellent communication and empathy with patients.
• Teamwork, continuous learning, and high-quality care.
• E-ClinicalWorks EMR experience is a plus.

Job description

We are seeking a compassionate and skilled Advanced Practice Provider (NP or PA) to provide comprehensive primary care services to patients across the lifespan. What You’ll Do:

• Conduct comprehensive physical exams, assessments, and patient histories


• Diagnose and manage a wide range of conditions


• Provide primary care support across multiple specialties


• Perform in-scope procedures


• Collaborate closely with specialists in ENT, Surgical, and Medical Subspecialties to ensure continuity and coordination of care


• Utilize E-ClinicalWorks for efficient, compliant, and patient-centered documentation


• Provide emergency and urgent interventions when necessary


• Educate patients and families on treatment plans, prevention strategies, and lifestyle modifications

Requirements

• Certified Nurse Practitioner (NP-C/FNP) licensed in the state of Ohio


• Must be experienced in primary care, family medicine, or internal medicine


• Comfortable managing a broad patient panel across all ages and backgrounds


• Proficient in clinical procedures relevant to outpatient care


• Strong communication and empathy skills—your ability to connect with patients matters as much as your clinical expertise


• Commitment to teamwork, continuous learning, and high-quality care
