Hometown Urgent Care & Occupational Health in Ohio seeks an Advanced Practice Provider (NP or PA) to deliver comprehensive primary care across all ages. You will diagnose, treat, and educate patients in a collaborative, team-based setting.

Experience in primary care and proficiency with E-ClinicalWorks are valued. This role emphasizes prevention, patient education, and coordinated care with specialty teams.

Qualifications

• Licensed in Ohio as NP-C/FNP or PA-C.
• Experience in primary care, family medicine, or internal medicine.
• Able to manage a broad patient panel across ages and backgrounds.
• Proficient in outpatient clinical procedures and EMR use.

Responsibilities

• Conduct comprehensive exams, histories, and assessments.
• Diagnose and manage diverse conditions in primary care.
• Support primary care across multiple specialties.
• Perform in-scope outpatient procedures.
• Collaborate with ENT, Surgical, and Medical subspecialists for continuity.
• Document efficiently in E-ClinicalWorks and ensure compliance.
• Provide emergency and urgent interventions when needed.
• Educate patients and families on plans and prevention.

Job description

We are seeking a compassionate and skilled Advanced Practice Provider (NP or PA) to provide comprehensive primary care services to patients across the lifespan. What You'll Do

• Conduct comprehensive physical exams, assessments, and patient histories
• Diagnose and manage a wide range of conditions
• Provide primary care support across multiple specialties
• Perform in-scope procedures
• Collaborate closely with specialists in ENT, Surgical, and Medical Subspecialties to ensure continuity and coordination of care
• Utilize E-ClinicalWorks for efficient, compliant, and patient-centered documentation
• Provide emergency and urgent interventions when necessary
• Educate patients and families on treatment plans, prevention strategies, and lifestyle modifications

Requirements

• Certified Nurse Practitioner (NP-C/FNP) or Physician Assistant (PA-C) licensed in the state of Ohio
• Must be experienced in primary care, family medicine, or internal medicine
• Comfortable managing a broad patient panel across all ages and backgrounds
• Proficient in clinical procedures relevant to outpatient care
• Strong communication and empathy skills - your ability to connect with patients matters as much as your clinical expertise
• Commitment to teamwork, continuous learning, and high-quality care
• E-Clinical Works EMR experience a PLUS!
