Job summary

Hometown Urgent Care in Wooster, OH is seeking a compassionate Advanced Practice Provider (NP or PA) to deliver comprehensive primary care across the lifespan in a fast-paced, collaborative setting.

You will conduct exams, diagnose and manage conditions, perform procedures, and coordinate with ENT, Surgical, and Medical subspecialties, using E-ClinicalWorks for documentation; Ohio license required; NP-C/PA-C; benefits and retirement options available.

Qualifications

• Must be an NP-C/FNP or PA-C licensed in Ohio.
• Experience in primary care, family medicine, or internal medicine.
• Comfortable managing a broad patient panel across all ages.

Responsibilities

• Conduct comprehensive physical exams and patient histories.
• Diagnose and manage a wide range of conditions.
• Provide primary care support across multiple specialties and perform in-scope procedures.
• Collaborate with ENT, Surgical, and Medical Subspecialties for continuity of care.

What You’ll Do:

• Conduct comprehensive physical exams, assessments, and patient histories
• Diagnose and manage a wide range of conditions
• Provide primary care support across multiple specialties
• Perform in-scope procedures
• Collaborate closely with specialists inENT, Surgical, and Medical Subspecialtiesto ensure continuity and coordination of care
• UtilizeE-ClinicalWorksfor efficient, compliant, and patient-centered documentation
• Provide emergency and urgent interventions when necessary
• Educate patients and families on treatment plans, prevention strategies, and lifestyle modifications

• CertifiedNurse Practitioner (NP-C/FNP) or Physician Assistant (PA-C)licensed in the state of Ohio
• Must be experienced in primary care, family medicine, or internal medicine
• Comfortable managing a broad patient panel across all ages and backgrounds
• Proficient in clinical procedures relevant to outpatient care
• Strong communication and empathy skills- your ability toconnectwith patients matters as much as your clinical expertise
• Commitment to teamwork, continuous learning, and high-quality care
