Job summary

Hometown Urgent Care & Occupational Health seeks a compassionate Advanced Practice Provider (NP or PA) to deliver comprehensive primary care across the lifespan in a team-based setting.

Responsibilities include exams, diagnoses, preventive care, procedures, and coordination with ENT, Surgical, and Medical subspecialties. Strong EMR experience with E-ClinicalWorks is a plus, with emphasis on patient education and high-quality care.

Qualifications

• Licensed NP-C/FNP or PA-C in Ohio.
• Experience in primary care, family medicine, or internal medicine.
• Comfortable managing a broad patient panel across all ages.

Responsibilities

• Conduct comprehensive physical exams, assessments, and patient histories.
• Diagnose and manage a wide range of conditions.
• Provide primary care support across multiple specialties.
• Perform in-scope procedures.
• Collaborate with ENT, Surgical, and Medical Subspecialties to ensure continuity of care.
• Utilize E-ClinicalWorks EMR for documentation.
• Provide emergency and urgent interventions when necessary.
• Educate patients and families on treatment plans, prevention strategies, and lifestyle modifications.

Requirements

• Certified Nurse Practitioner (NP-C/FNP) or Physician Assistant (PA-C) licensed in the state of Ohio
• Must be experienced in primary care, family medicine, or internal medicine
• Comfortable managing a broad patient panel across all ages and backgrounds
• Proficient in clinical procedures relevant to outpatient care
• Strong communication and empathy skills - your ability to connect with patients matters as much as your clinical expertise
• Commitment to teamwork, continuous learning, and high-quality care
• E-Clinical Works EMR experience a PLUS!
