Job summary

Bluestone Physician Services in Minnesota is seeking a Nurse Practitioner or Physician Assistant to provide on-site, team-based primary care within Assisted Living and Memory Care communities.

You will manage a 200-patient panel, design your schedule, and collaborate with LPNs, RNs, social workers; a competitive salary of $125k–$170k with CME support and generous time off. The role emphasizes preventive, proactive care with data-driven decision-making and opportunities for professional growth.

Qualifications

• Graduate of an accredited Nurse Practitioner or Physician Assistant program.
• Board certified or board prepared through AANP, ANCC, or NCCPA.
• Licensed or eligible for licensure in the state’s nursing or medical board.
• Ability to obtain and maintain DEA certification as required.

Responsibilities

• Provide on-site, team-oriented primary care to patients within Assisted Living and Memory Care communities.
• Full-time providers care for patients Monday-Thursday; option for Fridays to be a clinical or an administrative day.
• Manage a panel of 200 patients (approximately 12-16 per/day with 14 being the avg.).
• Ability to design and manage your schedule.
• Develop and implement care plans appropriate to patients and their desires.
• Clinical support from Clinical Assistants and Care Coordination through LPNs, RNs and Social Workers.
• Access to diagnostics such as labs and x-rays as needed.
• Collaborate with community partners within home care, hospice, and other resources as appropriate.
