Job summary

Bluestone Physician Services is seeking a Nurse Practitioner (NP) or Physician Assistant (PA) to provide on-site primary care in Assisted Living and Memory Care communities around Maple Grove, MN. You will work with a multidisciplinary team to deliver preventative, proactive care tailored to residents.

Join a growing practice with flexible scheduling, a workload of ~200 patients, and collaboration with LPNs, RNs, social workers, and community partners.

Qualifications

• Graduate of an accredited Nurse Practitioner or Physician Assistant Education Program.
• Board Certified/Board Prepared through AANP, ANCC, or NCCPA.
• Licensed or ability to obtain licensure through the Board of Nursing or Board of Medical Practice.
• Ability to obtain and/or maintain DEA certification for the state of work.
• Proven ability to communicate effectively with strong verbal skills.
• Excellent interpersonal and customer service skills.
• Must have a valid and unrestricted driver’s license and minimum state-required liability auto insurance; local travel is required.

Responsibilities

• Provide on-site, team-oriented primary care to patients within Assisted Living and Memory Care communities.
• Full-time providers care for patients Monday-Thursday; option for Fridays to be a clinical or an administrative day.
• Manage a panel of 200 patients (approximately 12-16 patients per day).
• Ability to design and manage your schedule; develop and implement care plans.
• Clinical support from LPNs, RNs and Social Workers; access to labs and X-rays.
• Collaborate with community partners within home care, hospice, and other resources as appropriate.
