Job summary

Bluestone Physician Services is seeking a Nurse Practitioner or Physician Assistant to provide on-site, team-based primary care in Assisted Living and Memory Care communities around Minneapolis. This full-time role involves managing a patient panel, designing care plans, and collaborating with Clinical Staff to deliver proactive, high-quality care.

Qualifications

• Graduate of an accredited Nurse Practitioner or Physician Assistant Education Program.
• Board Certified/Board Prepared through AANP, ANCC, or NCCPA.
• Licensed or ability to obtain licensure through the Board of Nursing or Board of Medical Practice.
• Ability to obtain and/or maintain DEA certification as required by the state of work.
• Demonstrates strong verbal communication and customer service skills.
• Must have valid, unrestricted driver's license; local travel required.

Responsibilities

• Provide on-site, team-oriented primary care to patients in Assisted Living and Memory Care communities.
• Full-time schedule Mon-Thu, with option for Fri as clinical or admin day.
• Manage a patient panel (200 patients; ~12-16 patients/day).
• Design and manage your schedule; develop and implement patient care plans.
• Receive clinical support from assistants, care coordinators, RNs, and social workers.
• Access to diagnostics such as labs and X-rays as needed.
• Collaborate with home care, hospice, and other resources.
