Bluestone Physician Services is hiring a Nurse Practitioner or Physician Assistant to provide on-site, team-based primary care in Assisted Living and Memory Care communities around Minneapolis/St. Paul, MN.

You will manage a patient panel, design care plans, and collaborate with nursing and social work teams to deliver proactive, individualized care. A strong patient-focused approach and valid state licensure are required.

Formación

• Graduate of an accredited Nurse Practitioner or Physician Assistant Education Program.
• Board Certified/Board Prepared through AANP, ANCC, or NCCPA.
• Licensed or ability to obtain licensure in the state of work.
• Ability to obtain DEA certification as required by work state.

Responsabilidades

• Provide on-site, team-based primary care to patients in Assisted Living and Memory Care communities.
• Manage a panel of approximately 200 patients with an average of 12–16 visits per day.
• Design and manage your schedule to balance clinical and administrative duties.
• Collaborate with Clinical Assistants, LPNs, RNs and Social Workers for coordinated care.
• Access to in-house diagnostics (labs and X-rays) as needed.
• Coordinate with home care, hospice, and other community resources.
