Bluestone Physician Services seeks a Nurse Practitioner or Physician Assistant to provide primary care in Assisted Living and Memory Care communities around St. Paul, MN.

You will join a collaborative care team delivering proactive, person-centered care directly to patients in community settings. Responsibilities include managing a panel of patients, designing care plans, and coordinating with clinicians and social workers.

Formación

• Graduate of an accredited Nurse Practitioner or Physician Assistant program.
• Board certification or board eligibility through AANP, ANCC, or NCCPA.
• Licensed or eligible for licensure in the required state.
• Ability to obtain DEA certification for the state of practice.
• Strong communication and interpersonal skills.
• Valid driver’s license and auto insurance with local travel requirement.

Responsabilidades

• Provide on-site, team-based primary care in Assisted Living/Memory Care communities.
• Manage a patient panel (approx. 200 patients) with 12–16 visits per day.
• Design and manage individual care plans and schedules.
• Collaborate with LPNs, RNs, social workers, and care coordinators.
