Job summary

Bluestone Physician Services in Minnesota seeks Nurse Practitioners or Physician Assistants to provide on-site, team-based primary care in Assisted Living and Memory Care communities around Southwest Minneapolis. You will manage a panel of about 200 patients and design your schedule with clinical and collaborative support from the care team.

The role offers base pay of $125k–$170k USD, plus CME opportunities, unlimited PTO and holidays, comprehensive benefits, and a retirement plan with employer

Qualifications

• Graduate of an accredited Nurse Practitioner or Physician Assistant Education Program (NP/PA).
• Board Certified/Board Prepared through AANP, ANCC, or NCCPA.
• Licensed or eligible for licensure; DEA certification as required by the state of work.

Responsibilities

• Provide on-site, team-oriented primary care to patients in Assisted Living and Memory Care communities as a float provider.
• Full-time schedule with Mon-Thu days; Fridays may be clinical or administrative.
• Manage a panel of ~200 patients (approx. 12-16 per day).
• Design and manage individualized care plans and patient schedules.
