Job summary

Bluestone Physician Services seeks a Nurse Practitioner or Physician Assistant to deliver on-site primary care in Assisted Living and Memory Care communities around Eagan, MN. You will join a multidisciplinary team, manage a panel of ~200 patients, and design care plans aligned with patient wishes.

Full-time roles, clinic and admin day options, with support from clinical assistants, RNs, and social workers. Travel required within local communities; DEA certification may be needed.

Qualifications

• Graduate of an accredited Nurse Practitioner or Physician Assistant program.
• Board Certified/Board Prepared through AANP, ANCC, or NCCPA.
• Licensed or able to obtain licensure by state nursing or medical board.
• Ability to obtain/maintain DEA certification as required.

Responsibilities

• Provide on-site, team-oriented primary care in Assisted Living and Memory Care communities.
• Manage a patient panel of about 200 and schedule flexibly.
• Develop and implement individualized care plans.
• Collaborate with LPNs, RNs and social workers for clinical support.
• Access diagnostics as needed and coordinate with community partners.
