Job summary

Bluestone Physician Services is seeking a Nurse Practitioner or Physician Assistant to deliver on-site primary care in Assisted Living and Memory Care communities around Minneapolis, MN. You will work within a multidisciplinary team to design and manage individualized care plans, supporting patients with chronic conditions and coordinating with LPNs, RNs, and social workers.

The role offers full-time hours, with a Monday-Thursday schedule and potential for a Friday clinical or administrative

Qualifications

• Graduate of an accredited Nurse Practitioner or Physician Assistant education program.
• Board Certified/Board Prepared through AANP, ANCC, or NCCPA.
• Licensed or eligible to obtain licensure through the Board of Nursing or Board of Medical Practice.
• Ability to obtain and/or maintain DEA certification as required by the state.
• Strong communication with patients and families; excellent interpersonal skills.

Responsibilities

• Provide on-site, team-based primary care in Assisted Living/Memory Care communities.
• Manage a panel of patients with a structured schedule (Mon-Thu; optional Friday).
• Develop and implement care plans in collaboration with the care team.
• Coordinate care with LPNs, RNs and Social Workers.
• Utilize diagnostics such as labs and X-ray as needed.
• Collaborate with community partners in home care, hospice, and other resources.
