Job summary

Bluestone Physician Services is hiring a Nurse Practitioner or Physician Assistant to provide on-site primary care in Assisted Living and Memory Care communities around Maple Grove, MN. You will manage a panel of about 200 patients and collaborate with Clinical Assistants, RNs, and Social Workers to design patient-centered care plans.

Full-time schedule is Monday to Thursday, with an option for Friday as a clinical or administrative day. Competitive base pay $125k-$170k plus benefits.

Qualifications

• Graduate from an accredited Nurse Practitioner or Physician Assistant Education Program.
• Board Certified/Board Prepared through AANP, ANCC, or NCCPA.
• Licensed or ability to obtain licensure in the state.
• DEA certification required where applicable.
• Effective communication with strong verbal skills.
• Excellent interpersonal and customer service skills.
• Valid unrestricted driver’s license; local travel required.

Responsibilities

• Provide on-site, team-oriented primary care in Assisted Living and Memory Care communities.
• Full-time schedule: Monday–Thursday; option for Fridays for clinical or admin day.
• Manage a panel of approximately 200 patients.
• Design and manage your schedule.
• Develop and implement care plans aligned with patient goals.
• Clinical support from LPNs, RNs and social workers.
• Access to diagnostics such as labs and x-rays as needed.
• Collaborate with home care, hospice and other resources.
