Job summary

Fairview Health Services seeks an In Clinic Visit Facilitator for the Primary Care team in Bloomington, MN. This 0.95 FTE, 76 hours per two-week period, role supports in-person visits, pre-visit communication, and documentation in the medical record.

The position assists with rooming, supplies, scheduling, and ensuring an efficient visit experience for patients and providers. Strong customer service and tech comfort are required; nursing or allied studies preferred.

Qualifications

• 1 year of customer service experience is required.
• Proficient with technology (computer hardware and programs, smart devices).
• Healthcare terminology familiarity is a plus.

Responsibilities

• Prepare patients for appointments and gather/record medical information.
• Assist in rooming and documenting visit details, vitals, and intake forms.
• Stock exam rooms and medical supplies; ensure a smooth patient flow.
• Support scheduling and coordination of follow-up appointments when needed.
• Communicate with patients, providers, and staff to improve the care experience.
