Job summary

Fairview Health Services is hiring an In-Clinic Visit Facilitator in Edina, MN to support the Primary Care team. This full-time role (1.0 FTE, 80 hours every two weeks) includes helping patients prepare for visits, collecting information for the medical record, stocking exam rooms, and coordinating follow-up appointments.

The position emphasizes a positive patient experience and effective communication with patients and providers.

Qualifications

• 1 year of customer service experience.
• Proficient with technology (computers and devices).
• Nursing/medical student or related coursework preferred.

Responsibilities

• Prepare patients for appointments and gather medical information for the visit.
• Assist in rooming patients and document vitals, medications, and health questionnaires.
• Prepare exam rooms and stock supplies.
• Facilitate patient-provider connection and ensure a positive visit experience.
• Assist with scheduling and coordination of follow-up appointments as needed.
• Communicate effectively with patients and health professionals, in person and via virtual tools.
