Job summary

Fairview Health Services in Oakdale, MN is seeking an In Clinic Visit Facilitator to join our Primary Care team. This 1.0 FTE role supports in-person visits, helps gather patient information, and ensures exam rooms are stocked and ready.

You will assist with scheduling, communicate with patients before visits, and help provide a smooth patient experience with the provider. Benefits and hours are described in the posting.

Qualifications

• 1 year of customer service experience.
• Proficient with technology (computers and smart devices).
• Preferred: nursing, medical, social work, psychology or pharmacy student.
• Certifications such as NA, EMT, Health Unit Coordinator desired.
• Experience with Epic Health Records and telephonic customer interactions.

Responsibilities

• Prepare patients for appointments by collecting and verifying medical records.
• Assist in rooming and documenting reason for visit, vitals, med review.
• Prepare exam rooms and stock supplies.
• Support patient connection to provider and ensure positive experience.
• Complete visit wrap-up with after-visit summary and follow-up scheduling.
