Job summary

Fairview Health Services in Edina, MN is seeking an In Clinic Visit Facilitator to join the Vascular, Vein, and Wound care team. This 1.0 FTE role (80 hours per two-week pay period) supports in-person clinic visits, pre-visit communication, and documentation in the medical record.

The role helps prepare exam rooms, gather patient data, and coordinate appointments or follow-ups as needed, contributing to a smooth patient experience and efficient clinic operations.

Qualifications

• At least 1 year of customer service experience.
• Proficient with technology (computer hardware and programs, smart devices).
• Preferred: nursing, medical, social work, psychology, or pharmacy student.
• Preferred: NA, EMT, or HUC program.
• Experience with medical terminology, Epic Health Records, telephonic customer experience, and health-care setting.

Responsibilities

• Prepare patients for appointments, ensuring medical records are collected and available.
• Assist in rooming, collecting reason for visit, medication review, vitals, and health questionnaires.
• Prepare exam rooms and stock supplies.
• Support patient connection to provider for visit and ensure positive patient/provider experience.
• Complete visit wrap-up with after-visit summary and follow-up scheduling where needed.
• Communicate effectively with patients, providers, and health professionals, including via virtual technologies.
