Job summary

Fairview Health Services in Edina, MN is seeking an In-Clinic Visit Facilitator to join our Primary Care team as a 1.0 FTE (80 hours per two week pay period), benefits-eligible. Clinic hours are Monday-Thursday 7:00AM-6:00PM and Friday 7:00AM-5:00PM, with no weekends.

The Visit Facilitator will support in-person visits by preparing patients, collecting and documenting information, stocking exam rooms, and coordinating scheduling and follow-up as needed to ensure a smooth, patient-centered care

Qualifications

• Minimum 1 year of customer service experience.
• Proficient with technology and devices.

Responsibilities

• Prepare patients for appointments and verify medical records.
• Assist in rooming and gather pertinent patient information.
• Prepare exam rooms and stock supplies.
• Support connection to providers for the visit and ensure a positive patient experience.
• Assist with scheduling and coordination of follow-up appointments.
