Job summary

CINQCARE is seeking a Practice Success Partner who will own the relationship with an assigned panel of practices in value-based care contracts. This role uses contract performance data to drive a targeted action plan, becoming the primary contact and performance consultant for the practices.

You will work with enterprise analytics and regional leadership, translating data into improvement actions and coordinating cross-functional efforts to close performance gaps.

Qualifications

• Five+ years in a practice-facing, account management, or provider relationship role within value-based care, managed care, or provider organization.
• Demonstrated ability to read a contract performance report and convert it into a specific, prioritized action plan.
• Demonstrated ability to own a leadership-level relationship, including direct conversations with practice leadership and physicians.
• Working knowledge of value-based care contract mechanics: quality measures, risk adjustment, transitions of care, and medical expense drivers.
• Proficiency with Excel and standard office software; ability to learn payer portals, analytics platforms, and internal tracking tools.

Responsibilities

• Translate contract performance data into a specific, practice-level action agenda and determine priorities.
• Maintain a focused set of actions per practice and review with leadership on a fixed cadence.
• Own the relationship end to end as the single accountable point of contact for each assigned practice.
