Job summary

You will assess, plan, implement, and evaluate nursing care, collaborating with a multidisciplinary team to achieve optimal patient outcomes. The position is a full-time day shift with a weekend differential, at Banner Del E.

Qualifications

• Graduation from an accredited nursing program with an RN license.
• Current valid RN license in state of practice and BLS certification.
• Experience in acute care settings is preferred.

Responsibilities

• Assess patient physical, psychological, social, spiritual, and discharge needs.
• Plan and initiate an individualized care plan with patient/family input.
• Coordinate and supervise care across the health care team.
• Document assessments, plans, interventions and patient response accurately.
• Ensure continuity of care and effective communication among caregivers.
