East Side House seeks a Case Manager for the PSP program to support post-secondary pathway planning, track deliverables, internships, and job placement for young adults in the Bronx and Northern Manhattan.

Under the supervision of the Employment Coordinator, you will manage a caseload, conduct intake, coordinate with instructors and services, and connect participants to housing, education, and workforce resources to help achieve their goals.

Qualifications

• Bachelor’s degree in social work or related field or equivalent experience.
• Experience working with city systems (e.g., justice, public assistance, housing).
• Strong communication and teamwork abilities.
• Background checks and clearances required to work at ESH.

Responsabilités

• Caseload management of 50–100 participants per year with regular engagement.
• Intake, needs/barrier assessments, and individualized service plans.
• Attendance outreach and strategies to improve retention.
• Interdisciplinary coordination with instructors and program staff.
• One-on-one and small group counseling to support education and employment goals.
• Facilitate workforce readiness and life skills workshops.
• Referrals to local workforce systems and community resources.
• Recruitment and onboarding support for new cohorts; maintain participant files.
• Data management, notes, and reports; participate in data-driven reviews.
• Attend team meetings and contribute to program improvement.
