Job summary

Career Team is hiring a Case Manager to support the Pathways for Access to Careers and Employment Workforce Development Program (PACE) on a 100% onsite basis in New York. You will provide intensive case management to guide clients in education and training pathways leading to employment and self-sufficiency.

The role emphasizes a human-centered approach, portfolio management, and collaboration with stakeholders.

Qualifications

• Bachelor’s degree in social work, social services, counseling, business administration, or related field.
• Or a minimum of 2 years of workforce development-related case management experience with a professional certification.
• Experience working with populations similar to the PACE program’s clients.

Responsibilities

• Manage a portfolio of clients by motivating their education, training, and job search efforts.
• Coach clients through barrier removal and professional development.
• Develop and manage relationships with stakeholders, including government agencies and employment partners.
• Identify and mitigate barriers to participation (health, housing, childcare, etc.).
• Document case management activities and prepare reports.
