Job summary

Commonpoint Queens is seeking a Case Manager for the Opportunity Youth Programs to support out-of-school youth ages 17–24. You will manage a caseload of 25–30 participants, conduct intake, assess eligibility, and coordinate wraparound services with community partners to advance education, employment, and life skills.

Qualifications

• BA in Human Services or related field preferred or required.
• 3+ years of case management experience with opportunity youth.
• Demonstrated ability in youth development and cultural competency with diverse populations.
• Experience with participant navigator services including public assistance, healthcare, financial, legal and social support.

Responsibilities

• Manage a caseload of 25–30 active participants and maintain follow-up with program graduates.
• Maintain communication with participants, staff, and support systems.
• Facilitate wraparound services to address educational, vocational, and personal needs.
• Conduct intake assessments and collect eligibility documentation.
• Develop and monitor Individual Service Strategies (ISS) aligned with participant goals.
