Job summary

Highland Park Community Development Corp in Bronx, NY is hiring a Case Manager to provide direct casework services to clients and residents, maintain case notes, and manage an assigned caseload with timely referrals.

The role reports to the Director of Social Services, requires a Bachelor’s in Social Work or related field, and is a full-time position with a standard Monday–Friday schedule and occasional travel.

Qualifications

• Bachelor’s Degree in Social work or related field.
• Certification in First Aid, CPR.
• 2 years related experience.
• Experience with homeless population, MICA, or Veterans community.
• Proficient in Word, Excel, Power Point, CARES, etc.
• Bilingual is preferred.

Responsibilities

• Provide direct casework services to clients/residents.
• Arrange meetings to assess basic needs and develop a service plan.
• Carry an assigned caseload of residents/clients.
• Complete required paperwork, case management notes and referrals timely.
• Resolve incident reports and complaint reports as required.
• Participate in supervision meetings and staff trainings.
