Job summary

Urban Pathways is seeking a dedicated Case Manager in New York City to deliver clinical and case management services for assigned clients. You will listen actively, document thoroughly, and contribute to an inclusive anti-racist culture while coordinating housing, entitlements, and employment resources.

The role requires collaboration with the Director of Social Services and entails field work, client intake, and ongoing communication with diverse populations.

Qualifications

• Bachelor’s degree required; Social Work or related field preferred.
• Minimum one year of case management, individual or group counseling experience preferred.
• Knowledge of NYC housing subsidy programs.
• Experience with people experiencing homelessness, mental illness and/or substance abuse population preferred.

Responsibilities

• Provides comprehensive case management services for assigned clients including referrals and resources (housing, entitlements, employment/education).
• Supports client objectives and promotes housing stability and independence.
• Conducts home visits, escorts clients to appointments, and conducts outreach as needed.
• Maintains timely documentation, intake processes, and updates in client records.
• Assesses client needs, develops service plans, and monitors progress.
• Engages in crisis intervention, client advocacy, and client group facilitation.
