Job summary

CAMBA seeks a Case Manager to support high-risk young adults at the Van Dyke Cornerstone Center in New York. You will build Individual Service Plans addressing education, employment, health, and life skills; meet weekly with participants; coordinate with partners; and participate in clinical supervision to ensure safe, effective service delivery.

The role requires an associates degree in human services and at least two years of related experience, with strong communication and documentation

Qualifications

• Associates degree in human services, psychology, counseling, or a related field.
• Minimum of two years of case management experience or related community work.
• Experience with high-risk, justice-involved, or system-impacted young adults preferred.
• Strong communication, documentation, and organizational skills.
• Proficiency with Microsoft Office and case management database systems.

Responsibilities

• Practice Universal Precautions/Standard Protocol & Procedures.
• Comply with all policies to protect confidentiality of health information.
• Ensure safety and security of participants by following program protocols.
• Initiate contact and establish trust with high-risk participants aged 18–21.
• Holistically assess strengths and needs; develop ISP with goals for education, employment, health and life skills.
• Meet weekly with each participant to review progress and adjust goals.
• Identify trauma and support weekly Healing Circles with clinical supervisor.
• Make, track, and follow up on referrals to City agencies and providers.
• Serve as an expediter to ensure referrals result in connected services.
• Work with Youth Navigator on ISP implementation and outreach.
• Participate in LMSW supervision virtually twice per month and present cases.
• Provide crisis intervention and de-escalation following protocols.
• Foster positive relationships and support life skills development.
• Support families and connect them to resources.
• Maintain confidential case records and prepare monthly reports.
• Enter data in required databases and assist with site visits.
• Attend meetings and maintain professional boundaries.
