Job summary

Fairfield Memorial Hospital is seeking a compassionate Population Health Nurse Educator to join the Horizon Healthcare Clinic team in Fairfield, IL. The role focuses on chronic disease management, patient education, and care coordination to improve outcomes and reduce barriers to care.

Qualifications

  • Graduate of an accredited School of Nursing and active RN license in Illinois.
  • BSN preferred; certifications in chronic disease management or patient education are a plus.
  • Experience with diabetic education and population health initiatives is preferred.

Responsibilities

  • Coordinate care and communication among patients, providers, caregivers, and community resources.

NATURE OF WORKFairfield Memorial Hospital is seeking a compassionate, proactive, and patient-focused Population Health Nurse Educator to join our Horizon Healthcare Clinic team. The Population Health Nurse Educator serves as a vital link between patients, providers, caregivers, and community resources, helping individuals navigate the healthcare system and take an active role in managing their health. This position focuses on chronic disease management, patient education, care transitions, population health initiatives, and quality improvement efforts designed to enhance patient outcomes and reduce barriers to care.
