Job summary

Allegheny Health Network is seeking a Medical Assistant/LPN to support population health management, assist during clinical visits, and provide self-management coaching to patients.

You will perform pre-visit planning, chart reviews, rooming, documentation, and entry of orders under supervision, while maintaining inventory and in-house testing. Applicants should have a High School diploma or GED or completed MA program or LPN license, with at least two years MA experience.

Qualifications

• High School / GED or Completion of a Medical Assistant Program or LPN License.
• Current PA LPN licensure OR multi-state licensure through eNLC.
• 2 years of Medical Assistant experience.

Responsibilities

• Population health outreach and closing gaps in care.
• Pre-visit planning with chart and data reviews.
• Assist provider during clinical visits and activate standing orders.
• Document clinical visit elements under supervision.
• Coach patients toward self-management using motivational interviewing.
• Perform registry and quality data reviews for population health.
• Coordinate care with care team nurses for high-risk patients.
• Maintain instrument sterilization and inventory.
• Conduct in-house testing following standards.
