Responsibilities

• Conduct population health outreach and care coordination.
• Prepare for visits by reviewing charts and data in advance.
• Assist with chart reviews and pre-visit planning.
• Perform in-house testing as directed.
• Maintain supplies and sterilize instruments per standards.
• Support adherence to HIPAA and privacy policies.
• Engage in patient education and self-management support.

Education
