Allegheny Health Network is seeking a Medical Assistant/LPN to support population health management and care coordination. You will assist during clinical visits, perform chart reviews, and help with data entry and order processing under provider supervision.

Qualifications

• High School / GED or completion of a Medical Assistant Program or LPN License.
• Current State of PA LPN licensure OR current multi-state licensure via eNLC OR Medical Assistant Certification.
• 2 years of Medical Assistant experience.

Responsibilities

• Actively participates in population health outreach through identification of patients with gaps in care using analytic tools and patient registries.
• Contacts patients and facilitates closing gaps in care.
• Ensures pre-visit planning by reviewing charts and data and preparing for patient arrival.
• Demonstrates expanded rooming protocol by assisting the provider in addressing care gaps and activating standing orders.
• Accurately performs clinical transcription during the patient encounter under supervision and assists with computerized provider order entry.
• Uses motivational interviewing to coach patients toward self-management and activation.
• Performs registry and quality data review and conducts population outreach to close gaps in care.
• Oversees identification of high risk patients and facilitates complex care management referrals.
• Reviews with care team to achieve population health goals and improve access.
• Cleans and sterilizes instruments and ensures inventory of medical supplies.
• Performs in-house testing and other duties as assigned.

Education
