Job summary

Qualifications

• Minimum High School/GED or completion of a Medical Assistant program or LPN license.
• 2 years of Medical Assistant experience.
• CPR certification.
• Experience with population health tools preferred.

Responsibilities

• Actively participates in population health outreach through identification of patients with gaps in care and follow-up.
• Ensures pre-visit planning by chart and data reviews before patient visits.
• Assists provider during visits by addressing care gaps and activating standing orders.
• Documents elements of the clinical visit under supervision and assists with order entry.
• Coaches patients toward self-management using motivational interviewing techniques.
• Performs registry and quality data reviews to close gaps in care.
• Oversees patient care coordination with care team nurses for rising risk patients.
• Reviews care goals with team to improve access to care and reduce ED visits.
