Job summary

Allegheny Health Network is seeking a Medical Assistant / LPN to support population health management, assist with clinical visits, and provide self-management coaching to patients. The role includes pre-visit planning, data reviews, and collaboration with care teams to address gaps in care.

Responsibilities include chart reviews, rooming under supervision, and conducting in-house testing while ensuring sterile environments and inventory levels.

Qualifications

• High School / GED or completion of Medical Assistant program or LPN license.
• Current PA LPN licensure OR current multi-state licensure via eNLC OR Medical Assistant Certification.
• 2 years Medical Assistant experience.
• CPR certification.
• Act 34 Criminal Background Clearance.
• Act 33 Child Abuse Clearance.
• Act 73 FBI Fingerprinting Criminal Background Clearance.

Responsibilities

• Actively participates in population health outreach through identification of patients with gaps in care via analytic tools and registries.
• Ensures pre-visit planning by chart/data review and preparing for patient arrival.
• Demonstrates expanded rooming protocol by assisting provider in addressing care gaps.
• Performs clinical transcription under supervision and assists with order entry.
• Uses motivational interviewing to coach patients toward self-management and activation.
• Performs registry and quality data review and population outreach to close gaps in care.
• Oversees high-risk patient identification and facilitates complex care management referrals.
• Reviews and assesses with care team members to improve access and reduce avoidable ED visits.
• Cleans and sterilizes instruments; maintains inventory of supplies.
• Performs in-house testing following established standards.
• Performs other duties as assigned.
