Job summary

Allegheny Health Network is seeking a Medical Assistant/LPN to support population health outreach and coordination of care. You will assist during clinical visits, ensure pre-visit planning, and help with documentation and order entry under supervision.

The role focuses on patient coaching for self-management, data-driven care gap closure, and collaboration with care teams to improve access and outcomes.

Qualifications

• Minimum High School / GED or completion of a Medical Assistant Program or LPN license.
• 2 years of Medical Assistant experience.
• CPR certification and clear background checks as required.

Responsibilities

• Assist with population health outreach and care gap identification.
• Prepare patients for visits by reviewing charts and data in advance.
• Assist providers during visits and activate standing orders as directed.
• Document clinical encounters under supervision and input orders accordingly.
• Coach patients toward self-management and follow-up on care plans.
• Review registries and perform data quality checks to close gaps.
