Job summary

Qualifications

• High School / GED or completion of a Medical Assistant Program or LPN license.
• Current State PA LPN licensure OR Multi-state licensure through eNLC OR Medical Assistant Certification.
• 2 years of Medical Assistant experience

Responsibilities

• Actively participates in population health outreach through identification of patients with gaps in care using analytic tools and registries.
• Performs pre-visit planning by chart/data reviews and preparing for patient arrival.
• Assists provider with rooming and activating standing orders to address care gaps.
• Accurately documents the clinical visit elements under supervision and assists with order entry.
• Coaches patients toward self-management using motivational interviewing; sets goals and reviews after-visit summaries.
• Reviews registry and quality data and conducts population outreach to close gaps in care.
• Identifies high-risk patients and facilitates complex care management referrals with care team nurses.
• Collaborates with care team to improve patient access and reduce ED visits.
• Cleans and sterilizes instruments, manages inventory, and stocks treatment areas.
• Performs in-house testing following established standards.
• Performs other duties as assigned or required.
