Job summary

Allegheny Health Network is seeking a Medical Assistant to support population health management, assist during clinical visits, and provide self-management coaching to patients with chronic conditions.

The role includes chart reviews, pre-visit planning, data entry under supervision, and collaboration with care teams to close care gaps. CPR certification and background clearances are required.

Qualifications

• High School / GED or completion of a Medical Assistant program or LPN license.
• 2 years of Medical Assistant experience.

Responsibilities

• Actively participates in population health outreach through identification of patients with gaps in care through analytic tools and patient registries.
• Performs pre-visit planning by chart and data reviews and preparing for patient arrival.
• Assists provider by addressing care gaps during visits and activating standing orders.
• Documents clinical visit elements under supervision and assists with order entry.
• Uses motivational interviewing to coach patients toward self-management and activation.
• Performs registry and quality data review and outreach to close gaps in care.
• Identifies high risk patients and helps with complex care management referrals.
• Reviews with care team to improve access to care and reduce ED visits.
• Cleans and sterilizes instruments and ensures supplies are stocked.
• Performs in-house testing and other duties as assigned.

Education
