Job summary

Allegheny Health Network is seeking a Medical Assistant to support population health management through patient outreach, care coordination, and assisting during clinical visits. You will help identify gaps in care and engage patients to achieve better outcomes.

You will perform chart reviews, assist with pre-visit planning, rooming, and order entry under provider supervision, and coach patients on self-management and activation. Strong teamwork is essential.

Qualifications

• High School / GED or completion of a Medical Assistant program or LPN license.
• Current PA LPN license OR current multi-state licensure through eNLC OR Medical Assistant certification.
• 2 years of Medical Assistant experience.

Responsibilities

• Identify patients with gaps in care using analytic tools and registries.
• Complete pre-visit planning with chart and data reviews.
• Assist with rooming and activating standing orders to address care gaps.
• Accurately document clinical encounters under supervision and assist with order entry.
• Coach patients on self-management using motivational interviewing and set goals.
• Perform registry and quality data review to close gaps in care.
• Identify high-risk patients and coordinate care referrals with care team nurses.
• Collaborate with care team to improve access and reduce avoidable ED visits.
• Cleans and sterilizes instruments and maintains inventory of supplies.
• Perform in-house testing following established standards.
• Perform other duties as assigned or required.
