Job summary

Allegheny Health Network is seeking a Medical Assistant/Care Coordinator to support population health management and patient outreach. You will assist with clinical visits, coordinate care, and provide self-management support under supervision.

This role requires high school completion, MA program or LPN licensure, with CPR and background clearances. Excellent communication and teamwork are essential as you work closely with providers and the care team.

Qualifications

• Minimum High School / GED or completion of a Medical Assistant Program or LPN License.
• Current State of PA LPN licensure OR multi-state licensure via eNLC OR Medical Assistant Certification.

Responsibilities

• Assists with population health outreach and care gap closure.
• Performs pre-visit chart/data reviews and prepares for patient visits.
• Assists provider by addressing care gaps and activating standing orders.
• Documents clinical visit elements and assists with order entry under supervision.
• Coaches patients using motivational interviewing for self-management.
• Reviews registry and quality data to close care gaps.
• Identifies high-risk patients and coordinates complex care referrals.
• Collaborates with care team to improve access and reduce ER visits.
• Cleans/sterilizes instruments and manages inventory.
• Performs in-house testing per standards.
• Other duties as assigned.
