Qualifications

• High School / GED or Completion of a Medical Assistant Program or LPN License.
• Current State of PA LPN licensure OR Current multi-state licensure through the enhanced Nurse Licensure Compact (eNLC) OR Medical Assistant Certification.
• 2 years of Medical Assistant experience.

Responsibilities

• Actively participates in population health outreach and care coordination.
• Assists with pre-visit planning and data reviews to prepare for patient visits.
• Supports the provider by expanding rooming protocols and standing orders.
• Documents clinical visit elements under supervision and assists with order entry.
• Uses motivational interviewing to coach patients toward self-management and activation.
• Performs data review and supports care coordination for high-risk patients.
• Maintains instrument sterilization and inventory of supplies; supports in-house testing.
