Job summary

Allegheny Health Network seeks a population health specialist to support outreach, gap closure, and care coordination. You will assist the provider during visits, perform chart reviews, and help with pre-visit planning to optimize patient care outcomes.

By applying motivational interviewing, you will coach patients toward self-management and activate them after visits, while collaborating with the care team on complex cases and data-driven improvements.

Qualifications

• High School diploma or GED or completion of a Medical Assistant program or LPN license.
• 2 years of Medical Assistant experience.
• CPR certification required.
• Clearances: Act 34/33/73 and FBI fingerprinting as applicable.
• HIPAA and privacy compliance awareness.

Responsibilities

• Outreach to identify gaps in care and close them.
• Assist with pre-visit planning and chart/data reviews.
• Verify rooming protocol and support the provider during visits.
• Document clinical visits under supervision and support CPOE.
• Use motivational interviewing to promote self-management.
• Coordinate care with care team nurses for high-risk patients.
• Review population health data and assist with referrals.
• Maintain supplies and perform in-house testing per policy.
