Job summary

Comprehensive Rehab Consultants in Harrisburg, PA is seeking a Nurse Practitioner/Physiatry clinician to deliver PM&R services in a designated post-acute SNF setting. You will work with an interdisciplinary team to promote patient independence and high-quality care.

The role offers 40 hours/week, regular hours around 9 AM–4 PM, a comprehensive benefits package, and opportunities for career advancement within CRC’s growing practice.

Qualifications

• Passion for post-acute healthcare and geriatric care.
• SNF or post-acute experience preferred.
• Ability to work autonomously and adapt to change.

Responsibilities

• Provide PM&R/Physiatry services as a rehab consultant in SNF/post-acute settings.
• Collaborate with interdisciplinary care teams and facility partners.
• Support documentation and coordination with PT/OT/SLP teams to improve patient function.
