Job summary

Comprehensive Rehab Consultants seeks a Part-time NP/PA/PM&R clinician to provide specialized PM&R services at a designated SNF/post-acute rehab center in Bloomfield, NM. You will collaborate with the rehab team, physicians, and facility staff to optimize patient outcomes and independence.

The role offers a flexible 8-hour shift (8AM–5PM), 16 hours weekly, full malpractice coverage, CEU opportunities, a 401(k) plan, and laptop. Remote options are not specified.

Qualifications

• NP/APRN/PA license required and current in NM.
• Experience in post-acute care or SNF preferred.
• PM&R interest and autonomy.

Responsibilities

• Provide PM&R/Physiatry services as rehab consultant.
• Collaborate with facility staff and interdisciplinary team.
• Support management of SNF conditions and rehab goals.
• Attend meetings and bridge therapy with internal medicine.
• Assist with documentation and care coordination.
