Comprehensive Rehab Consultants in Aurora, CO is seeking a PM&RP Clinician (NP/PA) to deliver specialized Physical Medicine and Rehabilitation services at a designated SNF or post-acute rehab center. You will collaborate with an interdisciplinary team and assume clinical leadership.

Responsibilities

• Provide PM&R/Physiatry services as a rehab consultant.
• Lead clinically within the facility and collaborate with the care team.
• Coordinate with PCPs for SNF-related conditions and surgeries.
• Work with PT/OT/SLP teams to improve patient function and quality of life.
• Track patient progress and barriers to therapy.
• Attend multidisciplinary meetings and bridge therapy with internal medicine.
• Engage in culture-building activities and ongoing education.
